Select a Form UAD.gif (26517 bytes)

Click 'Select Form.'  Select the applicable folder at the top.   Select the form.  Click 'OK' at the bottom.  The forms, addenda, sketch pages, map pages, photo pages, and forms for misc. documents are all available for selection.
